Transitional Care Unit
The Transitional Care Unit at St. Joseph Hospital is a short-term care, skilled-nursing facility for patients who no longer need the full resources of hospital care but who still require medical attention during their recovery or rehabilitation. A team of healthcare professionals, including physicians, nurses, social workers and activity specialists, provide care in the 20-bed unit, which provides a relaxed and homelike setting. The primary focus on the unit is to ready patients for their return home or to an appropriate community facility.
Located on the ninth floor of St. Joseph Hospital, TCU offers patients convenient and immediate access to acute-care facilities, laboratory tests, rehabilitation services, X-rays and other diagnostic and therapeutic services. The inconvenience of transferring to another facility for routine procedures is eliminated, resulting in improved recovery outcomes. The average stay for a TCU patient is 14 days.
Patients are encouraged to wear their own clothing and are invited to bring photos and other personal items from home to make their stay more pleasant. Friends and relatives are always welcome.
